Horizon Support Services is a Home Care Agency that is comprised of very helpful, well-trained, trustworthy, experienced caregivers. Our main goal is to provide you and your loved ones with the quality of life & peace of mind that you deserve.
We, at Horizon Support Services, are here because we realize that better care is offered through better understanding so we are dedicated to making life at home as enjoyable as possible and improving the level of independence for every client and family receiving our services.

Care Management:
Our support system is overseen by Care Managers who are Certified Gerontologists who furnish physical, emotional and spiritual aspects of caring involving the client and family in the planning stage.
Our Care Managers evaluate the client before developing a plan of care and services. A plan of care tailor-made to the individual's needs and preferences and health care issues. Care is provided by highly skilled caregivers, who have a minimum of two years experience working with seniors and are trained to carry out the support of the elders.
Care management includes:
- Supervision of caregivers
- Acts as a Liaison between caregivers and families
- Oversees client's plan of care
- Medication management
